Janice Vital

I created FormAlly Canada out of a passion for helping people confidently navigate the often-overwhelming world of forms and paperwork. Whether for personal, business, or legal purposes, I understand how intimidating and time-consuming these processes can be—and how much peace of mind comes from knowing they’re done right.

For years, I watched seniors, newcomers, busy professionals, and families struggle with government and institutional forms. These documents are often lengthy, filled with unfamiliar language, and sometimes available only online. For those who aren’t computer-savvy, are learning English, or simply don’t have the time, getting it right can feel impossible. Some even turn to lawyers for non-legal form assistance—paying hundreds of dollars for something that doesn’t require legal representation. Others, choosing to self-represent in court because they can’t afford a lawyer, often face setbacks when documents are rejected for using the wrong form or filling out the right one incorrectly.

With more than 25 years of administrative experience in education, government, and financial services, I worked daily with forms, applications, and documents. Every task demanded accuracy, discretion, and respect for privacy laws. I knew I had the skills to provide solutions that make paperwork simpler, more reliable, and more affordable—and so, FormAlly was born.

But I also recognized that true support had to go beyond just applications. Many of the same people who struggle with forms also face challenges with written English—whether drafting a website, preparing a business document, or writing important correspondence. That’s why I expanded FormAlly to include English language proofreading and copy editing, ensuring every client’s words are polished, professional, and tailored to make the right impression.

Supporting self-represented litigants became another natural step. To complement my form services, I trained in process serving through the National Investigative Training Academy (NITA), under the instruction of their Chief Academic Officer. This continuing education reflects my commitment to professional standards and staying current with industry best practices. No, we work with individuals, lawyers, law firms, government agencies, and businesses.
3. Are you a lawyer?
No, we are not lawyers and do not provide legal advice, opinions, or representation. Our role is to assist self-represented individuals with the administrative and procedural side of their paperwork.
4. Can you help with complex legal issues like wills or powers of attorney?
While we are not lawyers and can’t provide legal advice or representation, we are here to help with the administrative and procedural side of your paperwork. If we believe your situation would benefit from a lawyer’s expertise—especially for complex matters like wills, estates, or powers of attorney—we’ll let you know and help point you in the right direction.
5. Can you give me legal advice or help with a dispute or appeal?
No. We can explain the requirements and process for your forms, but we cannot interpret the law, tell you what legal strategy to take, or represent you in legal proceedings. If you need that type of help, we’ll recommend consulting a lawyer.
6. What types of forms can you help with?
We can assist with a wide range of forms, including: pension applications (CPP, OAS), citizenship and immigration paperwork, government benefits, insurance and investment applications, family law forms, and general administrative forms.
7. Do you fill out the forms for me?
Yes, we can either guide you step-by-step or complete the forms on your behalf, based on the information you provide.
8. Can you file my documents for me?
In most cases, yes. Depending on the form’s requirements, we can file them electronically, send them by mail on your behalf, or provide instructions so you can submit them yourself.
9. How do you ensure my information is safe?
We strictly follow Canadian privacy laws and take every precaution to keep your information secure and confidential.
10. How much do your services cost?
We offer flat-fee pricing so you know the cost upfront—no hourly billing or surprise charges. Pricing depends on the type and complexity of the form.
11. How much is your process serving fee?
We charge a flat rate of $75 for service within Hamilton proper (mileage included). For locations outside Hamilton proper, an additional $0.65/km applies.
12. How many attempts at service are included?
Our fee includes up to three attempts, plus preparation and signing of your affidavit(s) of service by a Commissioner of Oaths. Additional attempts, if required, are $40 each.
13. When do you make the first attempt at serving?
Our first attempt is made within 48 hours for addresses located in Hamilton proper.
14. Do you offer rush service?
Yes. Rush service means we’ll attempt service within 12 hours of your request. An additional fee of $40 applies.
15. Will you guarantee that my form will be approved?
No. We can make sure your forms are complete, accurate, and meet submission requirements, but final approval is always at the discretion of the reviewing agency.
16. What do you charge to file documents?
We charge $30 for online filing and $40 for in-person filing.
17. Do you file at out-of-town courthouses?
Yes, we do. We’ll be happy to provide a quote based on the specific court location.
18. Will you deal directly with the agency reviewing my form?
We can communicate with agencies to confirm submission requirements and processes, but we do not act as your legal representative or argue your case for you.
19. Do you only work with people in my area?
No, we work with clients anywhere through phone, e-mail or video meetings.
20. Are there any additional fees I should know about?
Service fees do not include court filing fees, which must be paid separately in advance. If we need to print hard copies of documents sent electronically, a $0.20/page fee applies. You will always be notified in advance if additional costs are required.
21. How do I get started?
